Hawaii TDI

Hawaii Temporary Disability Insurance (TDI) helps provide financial support when you need time away from work to recover from a non-work-related illness or injury, including pregnancy. If you qualify, you can receive partial wage replacement to help support you while you focus on your health and well-being.

Who Is Eligible?

You are eligible for the Hawaii State Disability program if you have at least 14 weeks of Hawaii employment in which you:

  • Were paid for 20 hours or more.
  • Earned no less than $400 in the 52 weeks preceding the first day of disability.
  • Are currently employed.

What Is Covered?

The Hawaii TDI plan covers 58% of your average weekly wages up to a maximum of $650 per week. 

How to Apply for Hawaii TDI

If you need time away from work because of a qualifying illness or injury, take these steps:

  • You will be required to submit completed Parts A and C of the Hawaii TDI claim form. Please contact LeaveManagement@bayada.com to retrieve the form for completion. 
  • Leave Management will complete Part B of the form. 
  • Completed Parts A and C can be emailed back to LeaveManagement@bayada.com or submitted to Tristar via email TDI.FI@tristargroup.net or fax (808) 470-0862. 
  • For phone inquiries regarding your claim, call (808) 470-0855.

Hawaii TDI and Your Leave of Absence

Hawaii TDI provides partial wage replacement while you are unable to work because of a qualifying disability. It is separate from other leave programs, which may have different eligibility requirements.
