BAYADA

Eligibility

Ready to explore your BAYADA benefits? We’re here to help you understand your eligibility and who you can cover. As you get ready to enroll, take note of important enrollment windows as a new hire or if you experience a Qualified Life Event. If you enroll dependents, remember to complete dependent verification by the deadline.

Benefits Eligibility

You and your eligible family members may participate in BAYADA’s employee benefits program if you meet the following eligibility requirements.

If you work an average 30 hours per week over a 3-month measurement period, you will be able to participate in BAYADA’s group health insurance, dental, and vision plans. If your weekly hours worked average below 30, you will still be able to sign up for one of four Minimum Coverage, Enhanced Minimum Coverage, Copay plan, or Copay Plus health insurance plans.

To qualify for and maintain group health insurance coverage, office employees must be regularly scheduled to work and meet the following minimum average hour requirements:

  • Part-Time Office Employees: Must be reguarly scheduled to work a minimum of 30 hours per week.

To maintain your benefits coverage, you must continue to meet the minimum weekly hour requirements for your Practice.

New Hires

As a new hire, you may choose to participate in BAYADA’s benefit plans. Some plans are available right away, while others become available after 60 days of employment at BAYADA.

  • Minimum Essential Coverage: Shortly after hire, you may enroll yourself and your eligible dependents in a Minimum Essential Coverage (MEC) plan. These plans are designed to help reduce your out-of-pocket costs when you need basic medical care and typically cost less than traditional medical plans. Coverage becomes effective on your date of hire and lasts for the duration of the plan year, unless you experience a Qualified Life Event.
  • Aetna medical plans: After 60 days of employment, you may choose to enroll yourself and your eligible dependents in an Aetna medical plan. Coverage becomes effective on the 90th day after your hire date and lasts for the duration of the plan year unless you experience a Qualified Life Event.
  • All Other Benefits: After 60 days of employment, you may enroll yourself and your eligible dependents in other BAYADA benefit plans that support your overall well-being, including Dental, Vision, Voluntary Life, Critical Illness Insurance, and other voluntary benefits.

Some benefits, such as Pet Insurance, may be available for enrollment sooner or at any time throughout the year. Speak with a Benefits Counselor to learn more about specific benefits and enrollment timelines.

Please note: To enroll your eligible dependents in a benefit plan, you must first enroll yourself in that plan.

Dependent Eligibility

Eligible dependents include:

  • Your legal spouse, domestic partner, or civil union partner.
  • Children under the age of 26, including natural children, stepchildren, legally adopted children, and children under legal guardianship, regardless of student, dependency, or marital status.
  • Unmarried children of any age who are fully dependent on you for support due to a disability and who are indicated as such on your federal tax return. Documentation of the child’s handicapped or disabled status must be provided. Please contact Aetna at 1-800-922-5863 to confirm eligibility.

IMPORTANT NOTE

You may enroll your legal spouse, domestic partner, or civil union partner in one of BAYADA’s Aetna medical plans only if they do not qualify for their own employer-sponsored medical coverage.

Coverage for an enrolled child ends on the last day of the month in which they turn age 26.

Domestic Partner Eligibility

You may enroll an eligible domestic partner or civil union partner in BAYADA benefits. After enrollment, your domestic partner will be included in BAYADA’s dependent verification process. You will receive an email directly from our verification vendor, Verifi1, with instructions on the documentation needed and next steps. Be sure to complete the verification process by the required deadline to avoid an impact to coverage.

If you enroll a domestic partner in coverage, imputed income may apply. The value of the coverage may be added to your taxable income each pay period, but you will not receive this amount as additional pay.

Dependent Verification

To ensure that only eligible dependents are enrolled in our Aetna medical plans, we conduct a Dependent Eligibility Verification (DEV) program. New dependents enrolled in the plan will be reviewed and verified by Verifi1, a third-party partner. Providing these documents helps ensure your dependents are properly enrolled and covered under your Aetna medical plan.

A partial list of documents you may be asked to provide includes:

  • Marriage certificate
  • Federal tax return
  • Birth certificate
  • Adoption certificate
  • Court order/QMCSO
