How to Report a Leave
If you know you will need time away from work, start the leave process as soon as possible. You should notify MetLife and your manager when you become aware that you may need a leave of absence. Failing to report a leave may result in an unprotected absence that could affect your job or BAYADA health insurance.
Reporting Your Leave
Step 1: Contact MetLife
Contact MetLife to report your leave and begin the leave process. Call 877-638-8262 to apply for leave or ask questions about your claim, including whether your leave has been approved or when you can expect payment.
When you report your leave, provide the information MetLife needs to determine which leave programs may apply to you. Depending on your situation, you may need to provide medical or other supporting documentation.
Step 2: Notify Your Manager
Let your manager know that you will need time away from work and, if known, when your leave is expected to begin and how long you expect to be out.
If your leave is unexpected, notify your manager as soon as possible and follow BAYADA’s usual absence-reporting procedures. For FMLA leave, federal law generally requires at least 30 days’ notice when the need for leave is foreseeable and 30 days’ notice is practical. If that is not possible, you should provide notice as soon as possible.
Step 3: Complete Required Documentation
MetLife will let you know what information or documentation is needed to process your leave. Respond to requests within the required time frames.
If your leave may qualify for FMLA, you may need to provide certification from your health care provider. You do not need to provide your diagnosis to your manager, but you must provide enough information for BAYADA to understand that your absence may qualify for protected leave.
Step 4: Stay In Touch During Your Leave
Keep MetLife and your manager informed if your leave dates change, your leave needs to be extended, or your expected return date changes.
If you are taking intermittent FMLA, you must report each time you use intermittent leave to both your manager and your MetLife claim manager.
Important Note
Report your leave as soon as you know you may need time away from work. You don’t need to have every detail figured out before contacting MetLife. Starting the process early can help ensure you receive the support and guidance you need.
It’s also important to stay in touch with MetLife and the BAYADA Leave Management Office and respond to requests for information within the required timeframes. Delays in providing information may affect your benefits or delay the processing of your leave request or disability claim.
