BAYADA

Eligibility

Ready to explore your BAYADA benefits? We’re here to help you understand your eligibility and who you can cover. As you get ready to enroll, take note of important enrollment windows as a new hire or if you experience a Qualified Life Event. If you enroll dependents, remember to complete dependent verification by the deadline.

Benefits Eligibility

Benefit eligibility for Hawaii office employees is based on your Class Field Value (CFV) and Hawaii-specific eligibility requirements.

Eligible populations include:

  • Full-Time Office: Exempt / Non-Exempt HR
  • Part-Time Office: Non-Exempt PT

New Hires

When Do I Become Eligible?

As a new hire, your eligibility for BAYADA’s medical, dental, and vision benefits depends on your employment classification and hours worked.

Full-Time Office Employees: Become eligible on the first of the month following four consecutive weeks of employment.

Part-Time Office Employees: Eligibility depends on whether your position is guaranteed or non-guaranteed. Guaranteed Part-Time employees become eligible on the first of the month following four consecutive weeks of employment. Non-Guaranteed Part-Time employees must work at least 20 hours each week for four consecutive weeks to become eligible on the first of the following month.

Once eligible, you must work at least 80 hours per month to maintain your medical, dental, and vision coverage.

When Can I Enroll?

Once you meet the eligibility requirements, BAYADA will notify you through Workday and email when you can enroll.

You have until the end of your eligibility month to enroll yourself and your eligible dependents in medical, dental, and vision coverage.

You may also be eligible for additional BAYADA benefits based on your employment classification, including life insurance, identity theft protection, 401(k), and voluntary benefits. Enrollment timelines may vary by benefit.

For assistance with enrollment or questions about your benefits, contact a BAYADA Benefits Counselor at 877-318-1764.

Please note: To enroll your eligible dependents in a benefit plan, you must first enroll yourself in that plan.

Dependent Eligibility

Eligible dependents include:

  • Your legal spouse, domestic partner, or civil union partner.
  • Children under the age of 26, including natural children, stepchildren, legally adopted children, and children under legal guardianship, regardless of student, dependency, or marital status.
  • Unmarried children of any age who are fully dependent on you for support due to a disability and who are indicated as such on your federal tax return. Documentation of the child’s handicapped or disabled status must be provided.

IMPORTANT NOTE

You may enroll your legal spouse, domestic partner, or civil union partner in one of BAYADA’s medical plans only if they do not qualify for their own employer-sponsored medical coverage.

Coverage for an enrolled child ends on the last day of the month in which they turn age 26.

Domestic Partner Eligibility

You may enroll an eligible domestic partner or civil union partner in BAYADA benefits. After enrollment, your domestic partner will be included in BAYADA’s dependent verification process. You will receive an email directly from our verification vendor, Verifi1, with instructions on the documentation needed and next steps. Be sure to complete the verification process by the required deadline to avoid an impact to coverage.

If you enroll a domestic partner in coverage, imputed income may apply. The value of the coverage may be added to your taxable income each pay period, but you will not receive this amount as additional pay.

Dependent Verification

To ensure that only eligible dependents are enrolled in our medical plans, we conduct a Dependent Eligibility Verification (DEV) program. New dependents enrolled in the plan will be reviewed and verified by Verifi1, a third-party partner. Providing these documents helps ensure your dependents are properly enrolled and covered under your medical plan.

A partial list of documents you may be asked to provide includes:

  • Marriage certificate
  • Federal tax return
  • Birth certificate
  • Adoption certificate
  • Court order/QMCSO
